style assigned to survey not showing as expected

hi

i have defined a style from scratch with an image in the header and footer and a background.

the preview style shows the images correctly

then I assigned the style to a survey

testing the survey shows the background but not the images in the header and the footer

How were the images added to the style template? Are they linked from another site or were they uploaded to the Checkbox server through the style editor interface?

they were uploaded from a directory on the machine where checkbox is running
