I am designing a controlled access survey (username/password required) and in testing today discovered that the log-in page respondents are directed to by the invitation does not reflect any of the style template settings I have set for the survey itself. Nor have I been able to determine where within Checkbox I can customize that page.

Where can I customize this page?
