Is it possible to attach a file to a Checkbox survey that the respondent can fill in and return with the survey?

What I have is a long list of questions that were developed in Excel and the group doesn't want to convert the questions in to the Checkbox tool, but want the respondents to answer and return. The asking about this know that the responses to the questions will not be part of the "reportable" database.

There isn't a way to directly accomplish with a single click of a button, but it seems like you could use a combination of existing survey items to make it work.

The first step would be to create an HTML item that contains a link to the spreadsheet you wish to distribute. If you are using Checkbox 4.5, the HTML editor has a "Document Manager" feature that will let you upload the spreadsheet to the server so that a respondent can download and edit it when responding to the survey.

Next, add an Upload File item so that the respondent can upload the completed spreadsheet when taking the survey.
