Calendar Option
I have users who would like to include a date field. Currently, the only way I know of doing this is by creating a dropdown with Month, second dropdown with day. I can't even put them right next to each other.
1. Is there a way for a formated calendar to pop open for a survey taker to select the date?
2. If not, is there a way to have both dropdowns be displayed on the same line?

You can apply formatting rules to single line text questions. If you select a date format users will need to enter a string in one of the following formats, mm/dd/yyyy or dd/mm/yyyy. Additional, if the "Use Date Picker" system setting is enabled respondents have the ability to select their response from a calender. The "Use Date Picker" option should be enabled by default; however, you can confirm this by navigating to Settings -> Preferences.

Regarding your formatting question, a Matrix item could be used to construct the question you described. The Matrix item type allows respondents to answer several related questions at once, or to provide feedback along several axes. For a more detailed explanation of Matrix items please refer to page 47 of the current release of the manual (Prezza Technologies | Documentation and Manuals).

Calendar Picker
I checked the properties and it is selected. How do I add it on the survey so participants can select the date?

When editing your single line text item you will need to specify either "Date (MM/DD/YYYY)" or "Date (DD/MM/YYYY)" for the Answer Format option.
