Templates for Login page

Hello.

Following the instructions in Page Layout Templates document I created my own layout and successfully deployed it. Looks good. The only problem now - how do I apply this template to the survey login page?
As well - how do I apply template to the page with a language selector? It looks like those pages inherit styles, but not the template.

Looks like no one used the custom Layout Templates? Another question where do i set translations for the language prompt page?
