Help with Ranking

One of the items I need to create in a survey is a ranking (1-11) of items in order of importance. I've tried several options, but have not found the right functionality yet. The best I've been able to find is a matrix with radio buttons, however that allows people to assign a ranking level more then once. Any ideas? Thanks!

What version of Ultimate Survey are you using?

If you select Add Item, then Add Rating scale(located at the bottom of item types) you will be able to get the functionality you are looking for. Is that option available for you?

Depending on the version of Ultimate Survey that you are using, when you add the rating scale to the matrix you'll see a checkbox titled "Each response in this set must be unique." When this is checked, that means that each answer in the rating scale can be used only once, which sounds like what you want.
