Cookie details
When I read the documentation, it said that when you set the survey permission to allow each respondent to complete only one survey, the application saves a cookie to the person's computer.
What is contained in the cookie? (IP Address? Invitation ID? or some combination?)
What if that person accesses the survey from a different computer? Home and Office computers?
If it's the IP address, what if your organization doesn't assign a permanant IP address to their users? What if an office computer's IP address changes every four or five days or on restart?
All of these issues effect the data...

The concerns you raise are valid. Because cookies are saved to the respondents machine you can not guarantee that a determined individual will only take the survey once. As you point out they could delete the cookie, or simply use a different machine.
If you need to guarantee that a user only takes a survey once it is our suggestion that you use registered users. In this situation all response tracking is handled server side within Checkbox.

The cookie name is configurable through the Application Settings screen. Stored in the cookie are the number of responses a particular respondent has entered for surveys he/she has taken.
In Checkbox, only a guid identifying the respondent is stored in the cookie.
