    Default Survey questions lost

We built a survey. We activated and sent it out. We have the documentation that 5 invitees have completed the survey. We have the survey identifed. But when we try to look at reports, we get back there are no questions attached to the survey. When we try to take the survey, it says there are no questions. What happened to the questions? All indicators say the survey exists. It wasn't deleted. All indicators say it was completed 5 times. But now nothing to run a report on. Thank you.

Do you have a link to the survey that you can send me so I can look into the issue?

    Default Survey questions Lost

The host server and our agency are behind firewalls so the immediate answer to your question is no. I'll have to see about getting it posted on our public site.

Okay, we can try a different approach. You mentioned:

"We have the documentation that 5 invitees have completed the survey."

Can you explain what you mean by that? What sort of documentation are you referring to?

    Default survey questions lost

When I look at the surveys list--the particular survey I'm interested in says there are 5 responses. When I go to the reports section-I click on the details button it gives me the date and time the surveys were completed. When I click on the summary button, it says there are no questions to review reports for. When I tried to edit the response (from the list of 5 responders), I got the survey is inactive or there is nothing available to edit. The survey is still active thru 5-23-08. When I try to review the survey it says the survey contains no items. When I invite myself to take the survey it says survey unavailable.

Would you be able to post screenshots somewhere on the web and send me a link to them?

Thank you for that. It would still be best if you could send links to screenshots. Would that be possible?

    try this. http://usabilitydev.lni.wa.gov/Ultim...y/default.html

Let me know what other screen shots might be helpful. I just thought of something. The customer said they deleted the responses and resent the survey. Would deleting the responses delete the questions? I am going to test that out to see what they might have done.

Deleting responses should never delete questions.

Unfortunately the link you sent did not work. It looks like it only allows connections from your server.
