Survey does not appear after login

I inherited Ultimate Survey Advanced 7.1 from a previous webmaster. I was recently notified this survey was no longer working. The previous webmaster fixed the connection string so the survey could access the SQL database.

However, upon a successful login, I only see the header for the page but nothing else. I think part of the issue is the survey not seeing the style sheet since there is no formatting on the page. Since I am not an ASP programmer, I do not know how to diagnose the issue and restore the functionality.

If the defendant does not answer the second question and click the Next, which receives the standard warning that he / she has not responded to a question mandatory and must return. It should be. Once the defendant hit the back button, even if the first question is visible, with the previous answer. The second question - who did not respond earlier - is not visible. The only visible way to do is answer the first question again. They say that the previous answer was 15. The 15 is still in the dropdown. For the second number appears, change the 15 to another number. Then comes the second question. To maintain the initial response, you must modify the new answer to the question for the first time in 15.
