Images not appearing and links not working due to incorrect web.config settings

When loading Ultimate Survey Enterprise survey software the images do not load and the links are not working properly and/or are going to the wrong place.

This is caued by incorrect settings in your web.config.

To remedy, confirm the following settings in the web.config:
1) ApplicationURL - This should be the URL to the root of your web server, e.g. http://www.domain.com

2) ApplicationRoot - This should be the web path to your installation e.g. /UltimateSurvey

Once you have changed these settings, simply retry the login.
