Health risk assessment

A Health Risk Assessment includes questions to assess the patient’s general health, as well as their attitude to health and wellbeing and their risk factors for specific scenarios, e.g. in the case of older patients, assessing the safety of their home for risk of physical injury.

A Health Risk Assessment could be used in the following cases:

  • To give individuals feedback about their health risks and what type of action they can take to improve their lifestyle, e.g. making changes to diet, stop smoking etc.
  • To assess health risks of a population e.g. collect data from employees at an organization to evaluate most common risk factors. Organizations could use this data to decide upon actions that should be taken to reduce health risks to their employees.
  • Healthcare enrollment programs often use a health risk assessment both when enrolling new members and on a yearly basis to examine changes over time and highlight new issues which need attention.

How do we gather health risk assessment data?

Health risk assessment data can easily and efficiently be gathered by asking a collection of questions as demonstrated in the template.

Most questions in a health risk assessment are short and do not require a high level of detail, so closed questions, either single-select or multiple-choice questions are recommended. These questions will gather quantitative data which is quick to analyze and easy to gain a general picture of the patient’s health from the answers.

If more detail is required, use open-ended questions with a comment box.

For an optimum user experience, carefully plan the objectives of the assessment and make sure to only ask essential questions. This keeps users engaged and reduces the drop-out rate.

Show respondents questions that are relevant to them e.g. if mobility around the home or memory function is only relevant to survey respondents over a certain age, add question logic so these questions are only shown to those who fit the criteria.

If your survey is more than one page long, include a progress bar so respondents can see that the survey is not overwhelmingly long and will be motivated to complete it.
