When creating a survey with multiple rating scales, keep the scales consistent. 

If the first question asks a respondent to rate your customer service on a scale of 1 to 5, with 1 being ‘very dissatisfied’ and 5 being ‘extremely satisfied’, make sure that in subsequent questions, 1 and 5 correspond to the same ratings.

This will avoid confusion with your respondents and will generate more accurate results.
