Signature
The signature question type is an incredibly useful tool that can simplify your work significantly. Here is a simple overview of how to use this feature.
When Do You Need a Signature?
This question type allows you to capture respondents’ signatures. This is useful for sending out forms and collecting user agreements in one spot and storing them in your system, along with collecting confirmation of compliance with the rules and policies.
Privacy laws vary by region so it is important to see what exact rules one must follow when using this question type. It is important to note that this can differ based on how you are hosting your data.
The following information will impact users who are hosting on Checkbox servers. No matter your location, Checkbox provides users with tools for compliance with various privacy rules, such as HIPAA, GDPR, and other privacy laws. How to Use The Signature Question ?
Let’s have a look at how to use this question to your advantage.
- Navigate to the “Add Question” panel and choose “Signature”.

- Write down instructions for the question in the text or subtext box.

- Create the rest of your questionnaire and then choose the placement of the signature question, so that it corresponds to your needs. Generally, this type of question goes to the end of your questionnaire since you would want the respondent to first read the content and then choose to sign or not.
Behavior
This section allows you to set different validations for your questions in addition to setting up an alias for your question.
Make Question Required
Check this box if you want the question to be answered as compulsory. The user can not go forward from the current page unless the question is answered.
Make Question Soft Required
Encourages the respondent to respond to the question. The user encounters the message “An answer is recommended, but not required” if the question is left unanswered in one attempt. Respondents can ignore the message and click “Next” again to go to the next page of the survey.
Question Alias
Aliases are the alternate texts or codes that you can display on your reports/exports. These values are not visible to the respondent and are helpful when you want to associate a particular value with your provided question.

Appearance
This section of your question allows you to administer how your questions and your options would appear on your survey page. You can also decide what color the signature will be drawn with.
Question Text Position
Lets you decide the position of your question with respect to the options. By default, it is set to “Top”. This means that the question will be placed above the answer options
Question position
Lets you decide the alignment of the question (left, middle, right) with respect to the survey page. By default, it is set to “Left. This means that your question will be left indented.
Width and Height
Adjust and height and the width of the signature box in pixels.
Color
You can use this option to change the color of the signature marker. By default it is set to #0000 (Black)

Conditions
This section of the question allows you to manage the conditional appearance of the question based on either a previous response, a contact attribute or a response property. You can combine 2 or more conditions using “OR/ AND” conditions by clicking on them. Please refer to our “Quick Start: Add a Condition to a Question”.

How Will The Question be Saved
Great! After you have collected your answers, you can view your stored signatures and export them. Simply navigate to the “Responses” tab and click on a specific respondent to view their signature, or export your responses to download the signatures in bulk. The signatures will be saved as .png files when exported.

Viewing Responses
The Signature responses are stored in Checkbox in the form of files. While these responses can not be viewed in the Report, you can certainly have a look at them in the responses.
An easy way to export all the signatures is to navigate to Survey -> Responses -> Export Responses. In the pop-up menu, scroll down to the Uploaded Files section. Specify the date range (if you want) and click on “Download files”. The signature files and all other attached files submitted by the respondents will begin to download in your local system on the backend.
